Take a Deep Breath
We are not in a hurry. The kind of health care we practice
takes time. Your first appointment will take between 60 and 90 minutes.
Your practitioner will be asking you lots of questions to really get
to know you and your health concerns. Together you will put together
a treatment plan. Your follow up visits are usually 30–60 minutes.
New Patient forms
Please download and fill out the below New Patient Intake Forms
before your first appointment. If you have questions, please call

"Day of" Info
- Our office staff will call you the day before your appointment
to remind you.
- Please avoid caffeine for three hours before your appointment.
- We have patients with chemical sensitivities so please
avoid wearing perfume or other scented lotions or hair products
on the day of your appointment. We appreciate it!
Cancellation Policy
If you need to cancel an appointment or change the time or date
please call as soon as possible. We charge for missed appointments
or those cancelled with less than 48 hours notice if we are unable
to fill your time slot from our waiting list.
Fees and Payment
Each practitioner at Hill Park has an individual fee schedule. Fees are based on the complexity of your needs and the length of time
spent with you. We ask for payment at time of service and accept personal
checks, Visa, MasterCard or cash. Please discuss any fee or payment
concerns with the front office staff.
Insurance
Currently some of our practitioners accept Workers Compensation
Insurance and Automobile Liability Insurance.
For all insurance carriers
we provide you with a superbill which you then submit
to your own health insurance company. We provide accurate diagnosis
and procedure codes to help increase your reimbursement levels. Most
health insurance companies reimburse you for the physician visit and
many do so for acupuncture.
Emergency and After Hours Care
We treat many kinds of health needs but our doctors are not on
call after hours and do not admit patients to the hospital. We
are happy to handle problems that arise during office hours but strongly
suggest that you have a primary care physician for urgent or after
hours medical problems. Thank you and we look forward to meeting you. |
